License Requirements:
All EPN employees will need to maintain State RN licenses and or any required certificates useful to the clients. Please plan accordingly. An expired license or out dated credentials immediately makes you non-employable.

Any caregivers with questions or concerns about any type of discrimination in the workplace are encouraged to bring these issues to the attention of their immediate supervisor or the owner.  Caregivers can raise concerns and make reports without fear of reprisal.  Anyone found to be engaging in any type of unlawful discrimination will be subject to disciplinary action, up to and including termination of employment.  After following proper protocol, any employee who feels their complaints of discrimination are not handled properly should contact the State Human Relations Commissions.  

Employment Applications 

Elite Premier Nurses relies upon the accuracy of information contained in the employment application, as well as the accuracy of other data presented throughout the hiring process and employment.  Any misrepresentations, falsifications, or material omissions in any of this information or data may result in the exclusion of the individual from further consideration for employment or, if the person has been hired, termination of employment.  

Employment Reference Checks 

To ensure that individuals who join EPN are professionally qualified and have a strong potential to be productive and successful.  It is the policy of EPN to check the employment references of all applicants.  Some positions require that Motor Vehicle record checks, TB tests, Physicals, and Criminal Record investigations be considered.  Motor Vehicle record, Background checks, Physicals and TB tests will be provided by the caregiver and conducted at the caregiver’s expense.  EPN will respond to all reference check inquiries from other employers.  Responses to such inquiries will be limited to information that can be substantiated by EPN records.  

Background Investigation Policy 

EPN strives to provide a safe and secure business environment for its clients and caregivers.  Third parties help to limit EPN liability exposure.  EPN policy is to conduct a thorough background investigation.  Social security number verification, medical physical, and drug screening on all applicants.  Furthermore, the applicant will not be permitted to begin employment with EPN until all required background, medical, and drug testing is completed and reviewed.  

We will evaluate background information consistently, according to non-discriminatory criteria.  Any criminal conviction or other negative information will be evaluated in relation to the position for which the applicant is applying for.  This evaluation may result in disqualification for consideration for employment at Elite Premier Nurses, LLC.  In addition, any falsification, or omission or information on any application form, including the background authorization, medical or drug authorization screening, and the employment application form may disqualify the applicant from consideration.  

In complying with the guidelines set forth by the PA Dept of Health, any applicant who cannot provide proof of residency in the state of Pennsylvania for more than two years will be subject to a background check on a Federal level 

All caregivers hired to work in homes where there are children under the age of 18 will also be subject to child abuse clearance investigation. 


 

The Client Bill of Rights 

As a client of Elite Premier Nurses, you have the right to: 

  • Be fully informed of all your rights and responsibilities 
  • Be treated with courtesy and respect 
  • Privacy, including confidential treatment of records and to refuse release of records to any individual outside of the company except in the case of the clients transfer to a health care facility or as required by law or third-party payment contracts. 
  • To be involved in the service planning process and to receive services with reasonable accommodation of individual needs and to receive appropriate and professional care 
  • Receive a timely response from the company to your request for service 
  • Be admitted for service only if the company can provide safe professional care at the level of intensity needed 
  • Receive reasonable continuity of care 
  • Be advised of any change in the plan of care, before the change is made 
  • Refuse treatment within the confines of the law and to be informed of the consequences of his or her action 
  • Be informed of your rights under state law to formulate advanced directives 
  • Be informed in writing 10 day prior to the anticipated termination of service or less than 10 days’ notice should you fail to pay for service and become more than 14 days in arrears of if the health and welfare of the direct care worker is at risk 
  • Be fully informed of company policies and charges for services, including eligibility for third-party reimbursements 
  • Be referred elsewhere, if denied service solely on your inability to pay 
  • Voice grievances and suggest changes in service or staff without fear of restraint or discrimination 
  • Be informed of what to do in the event of an emergency 
  • Be advised of the telephone number and hours of operation of the state’s home health hotline, which receives questions and complains and Medicare-certified and sate licensed home care agencies 

The client’s responsibilities are to: 

  • Express your care needs clearly 
  • Build mutual trust and cooperation with your caregivers 
  • Give relevant information to Elite Premier Nurses, LLC about your care needs 
  • Inform the EPN office of any major changes in your health status or condition 
  • Make a family member or substitute available who will assume a primary caregiver role when the EPN staff are not in your home.  Cooperate with EPN personnel without discrimination as to color, religion, sex, and national or ethnic origin.
